Credit Card Authorization Form
If you would like for your documents (I-20/acceptance letter, etc...) to be sent to you via Federal Express, or if would like to pay your fees, etc., please complete the credit card authorization form. Please check the appropriate box to indicate the reason for your credit card to be charged. Please be sure to sign and date this form, and then fax or hand-deliver to our office (unfortunately, we are unable to accept this form as an email attachment). For security reasons, we strongly recommend that you do not send this to us in electronic format.
