The School of Allied Health Professions has a reapplication process to allow students to continue on the program waiting list. By reapplying yearly, we can track students’ eligibility progress and can assure that we have the most current information.
Use the following electronic form to submit the following items before
Friday, January 20, 2012:
- REAPPLICATION FORM
- IF ANY PRE-REQUISITE OR GENERAL EDUCATION CLASSES WERE TAKEN DURING 2011 then
- Submit an Updated or new STUDENT EDUCATION PLAN (SEP) for the Associate of Science Degree. Use the standard SEP form-NOT the one from EOPS.

NOTE: Students who have submitted an SEP that indicates “GE Complete” do not need to submit another SEP. |

The appointment telephone number is (714) 432 5078 for a general counselor or (714) 432 5729 for the Allied Health counselor. |
And
- Submit updated copies of TRANSCRIPTS that show any new prerequisite or GE courses completed since your last submission.

Transcripts from Orange Coast College only require an unofficial advisement copy. |

Transcripts from other colleges require official sealed transcripts. |

Make sure your GRADES from the fall 2011 semester are included on all transcripts. |

Should you fail to return or complete the reapplication form and information requested by the deadline, your application will be withdrawn from further consideration for the program.
If you no longer want to be considered for a program, select the option ‘Remove my name from the waiting list’ and we will remove your name from the waiting list.
You will be notified of your status by mail during April 2012.
