SERVICE and LEADERSHIP APPLICATIONS
2014-2015 Academic Year
 
 
To qualify for a Service and Leadership Award, you must first meet all of the following minimum criteria:
  • Be enrolled in a minimum of 5 units in either the Fall 2014 or Spring 2015 semester.
  • Hold a cumulative grade point average of 2.0 or higher (some leadership awards require a higher grade point average; see reverse for more details).
  • Be in good standing with the Dean of Student Services
  • Paid the College Service Charge for the semesters in which you are/were enrolled.
  • Provided service and/or leadership at Orange Coast College for a minimum 20 hours during 2014-2015.
STUDENTS:
  • Fill out the application completely.  Incomplete applications will not be accepted.  This includes providing the name of the program for which you volunteered or were a leader, the name of your advisor, phonetic pronunciation of your entire name, etc.  Please write “N/A” if there is no information for that section.
  • Complete and submit an application for each service (volunteer) or leadership position in which you have been involved in 2014-2015.
     o You may email this form to your advisor so they may also fill it out electronically.  Be sure that the form is returned to the Dean of Student Services / ASOCC office before the deadline, either  to mdenunno@occ.cccd.edu or you may hand it into the office in person.
ADVISORS:
  • Fill out the application completely.  Incomplete applications will not be accepted.  This includes providing the name of the program for which the student volunteered or was a leader.
  • For leadership awards:
    o Mark only one of the three leadership award options
    o Include a brief written evaluation of this student’s service to OCC and how they have demonstrated their leadership ability.
DEADLINE:
Friday, March 13, 2015 at 12:00 p.m. (Noon).  Late applications cannot be accepted.

Levels and Criteria
 
Minimum Qualifications for all awards
  • 2.0 grade point average
  • Enrolled in 5 units in either the fall or spring semester
  • Be in good standing with the Dean of Student ServicesBe in good standing with the Dean of Student Services

Service Awards
 
Silver Certificate for Service
  • 20—69 hours of voluntary service to Orange Coast College
  • Minimum 2.0 grade point average at OCC
Gold Certificate for Service
  • 70 or more hours of voluntary service to Orange Coast College
  • Minimum 2.0 grade point average at OCC
Leadership Awards
 
Student Leader Award
  • Minimum of 70+ hours of voluntary service to OCC
  •  2.5 minimum grade point average at Orange Coast College
  • Meet at least one of the following criteria:
    o Organized an event, campaign, or activity
    o Taken a leadership role as documented and defined by the advisor (no documentation = no award)
Robert B. Moore Outstanding Student Leader Award
  • Minimum of 150+ hours of voluntary service to Orange Coast College
  • 3.0 minimum grade point average at Orange Coast College
  • Made a major contribution to OCC that affects large numbers of students or made a long-term change
  • Participated in at least two programs
  • Benefited current and future OCC students
Robert B. Moore Repeat Recipient (in addition to the above criteria …)
  • Must have a grade point average of at least 3.25 for the previous fall or spring semester
Joseph R. Kroll Student Leader of the Year Award and Scholarship
  • Qualify for a Robert B. Moore Outstanding Student Leader Award as defined above
  • and  Have a minimum 3.5 grade point averageHave a minimum 3.5 grade point average
  • and Have assumed a documented leadership role in at least two programs (“documented” can be in the form of advisor’s letter, any club/program records, etc.)
  • and Additional consideration will be given if you meet at least one of the following criteria
    o Been involved in several programs
    o Shown ongoing commitment through prior involvement
    o Exhibited evidence of academic excellence
Joseph R. Kroll Repeat Recipient (in addition to the above criteria …)
  • Must provide significant leadership role in at least one program that is different from the year for which you were previously awarded the Joseph R. Kroll Award.
