Enrollment Information

Our front office staff will contact each family regarding enrollment as space permits and direction will be given on how to return the enrollment forms.  Additional forms are required, for example, up-to-date immunization record and emergency card.  Prior to enrollment, a Parent Orientation will be scheduled and must be attended where requirements will be covered with acknowledgement by parent or guardian to abide by all policies and procedures in order for your child to participate in our program.  We are required to follow all Covid19 mitigation procedures in order to operate and these requirements are subject to change as directed.
